Identify the system type, manufacturer or model if known, affected components, when the problem began, and whether the condition is repeatable.

Operating condition

Explain whether equipment is stopped, running with restrictions, producing unusual noise or motion, or due for planned inspection or maintenance.

Field access and timing

Include plant location, working height, access equipment, shutdown limits, production schedule, and any site orientation or safety requirements.

Photos and records

Send clear overview and detail photos, equipment identification, recent inspection notes, drawings, parts information, and prior repair history if available.
